What is in the North Carolina file

Each row is one application reviewed by the NC Division of Health Service Regulation (DHSR): the applicant, the facility, the county, proposed and approved capital cost, the filing date and review cycle, competing applications, the decision and its date, conditions, appeals filed, and links to the state's decision letters. The busiest counties are Wake, Mecklenburg, Durham, Guilford.

Coverage, stated plainly

Decisions back to 2016 come from DHSR's yearly decision tables and letters. Filing dates, proposed costs and review cycles come from the application logs, which are public from 2019 on; the 2016 to 2018 logs are dead links on the state site, and a records request for them went in on October 28, 2026. The state does not post withdrawals.

  • North Carolina approves nearly every project as a conditional approval. The file records the conditions on each one.
  • A 2023 law took psychiatric and chemical dependency beds out of review and exempted surgery centers in counties over 125,000 people, so those filings thin out after 2023 by law, not as a gap.
  • Where the decision table and the letter disagree, the row keeps both and names each source.
